What you will do:
Complete initial and on-going interdisciplinary assessments with emphasis on determining level of function, and patient / family response to therapy. Includes OASIS assessments at appropriate time points.
Evaluate the level of function and initiate a plan of treatment for Occupational Therapy to assist with relieving pain, preventing disability, developing body skills, restoring function and maintaining maximum capabilities through the use of graded functional Occupational Therapy activities.
Initiate self-care modifications such as devising self-help eating methods, dressing and other daily living aids and instruct the patient and family in their use.
Develop, review and evaluate the plan of care in consultation with the physician and other interdisciplinary care team members, and in partnership with the patient, representative (if any), and caregiver(s).
Provide therapeutic treatments and evaluation of equipment needs that are ordered by the physician as indicated and reviewed by the interdisciplinary team.
Communicate with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.

Who you are:
Education and qualifications commensurate with § 484.115(f) (Condition of participation: Personnel qualifications / Occupational Therapist) as indicated by specific educational timeframes.
Graduate of an occupational therapy curriculum accredited jointly by the Council on Medical Education and the American Medical Assoc. and the American Occupational Therapy Association; and is licensed pursuant to ARS Title 32, Chapter 34
Core competency per skills checklist.
At least one year of experience in occupational therapy.
Criminal background, DMV and fingerprint card as appropriate.
Current CPR, TB and health clearance.
Complete 6 hours of in-service training per year.
