The base salary range for this position at commencement of employment is expected to be between $80,000 - $90,000; however, base pay offered may vary depending on multiple individualized factors, including job-related knowledge, licensure, skills, and experience.

JOB SUMMARY: Hospital Liaison is a professional representative of the Agency who functions as a health care educator and intermediary between the Agency and hospitals, facilities, health care professionals and the community. The purpose of this role is to facilitate an uncomplicated transition of patients to home care consistent with the mission and goals of the Agency.
DUTIES AND RESPONSIBILITIES
Assist in identifying appropriate patients through reviewing referrals, collaborating with hospital and case managers for patients to assist in securing appropriate Agency services and follow-up support
Helps to achieve and maintain continuity of patient care by planning and exchanging information with other health professionals within a hospital to home setting
Maintain relationships with Hospital, utilization review nurses, discharge planners, case managers, physicians and other health care professionals within a hospital
Provides appropriate education and marketing materials to patients, families, healthcare and hospital personnel in order to effectively promote utilization of Agency’s services
Maintains a level of knowledge of Agency services to present to hospital health care providers and payor sources
Attends and participates in rehabilitation, discharge planning and patient care meetings as requested
Provides and/or coordinates Agency sponsored educational opportunities and presentations
Meets regularly with the Branch Manager to discuss any potential hospital needs, to ensure high communication and follow through between the Agency and the assigned Hospital; and opportunities to provide higher level of customer service
Consults with Coordinators and Directors of Patient Care Services to ensure correct utilization of clinical services
Promotes positive working relationships with referral sources through effective communication and problem solving strategies to help facilitate appropriate service delivery
Prioritizes, organizes and manages workload according to the needs of clients and the goals of the Agency
Acknowledges patient rights including privacy and confidentiality
Consistently displays a professional appearance and conduct in all settings. Interpersonal and interagency relationships are to be positive and professional at all times
General availability to work during the office hours from 8am to 6pm with occasional weekends
Assumes responsibility and displays reliability for the completion of all tasks, duties, communications and actions
Maintains professional growth and development through networking, membership in professional organizations and continuing education
Participates in performance improvement activities
Abides by Agency policy and procedures
Accepts other duties as assigned
PHYSICAL DEMANDS AND WORKING ENVIRONMENT
Physically active, walking, standing and climbing stairs. May be exposed to minor hazards/communicable diseases. Must have visual and hearing acuity. Must be able to clearly communicate verbally. Must be able to read, write and speak English fluently. Must be able to lift weight equivalent to a box of computer paper. Must provide proof of auto insurance upon hire and each renewal and maintain a safe driving record.
EDUCATION, EXPERIENCE AND TRAINING
High School Diploma is required; Associate’s Degree or higher preferred
Current, active license as an LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners may be required based on Hospital assignment
A minimum of 2 years healthcare services, sales or related industry experience
Knowledge of hospital, physician, skilled nursing facility, case management needs
Ability to effectively communicate Agency’s services with all levels of the healthcare delivery team and clients
Must possess excellent organizational skills and be detail oriented
Must demonstrate strong written and verbal communication skills and be able to interact effectively with health care professionals, patients, peers and management
