We are hiring for a Program Manager for our Home Care [Private Duty) Department in Santa Barbara.


The pay range for this position at commencement of employment is expected to be between $75,000-80,000 annually; however, base pay offered may vary depending on multiple individualized factors, including job-related knowledge, licensure, skills, and experience.

What you will do:
  • Responsible for the planning, coordination and delivery of Private Duty services.  The Home Care Program Manager assists with the supervision of the Home Care team as it relates to the care and management of patients/clients, consistent with the mission and goals of the Agency. 
  • Responsible for referral, setup, training and coordination of all Home Care Program cases.
  • Will work with the Director of Patient Care Services to assess and admit appropriate patients for licensed Private Duty.
  • Interprets, informs, and enforces personnel and administrative policies including verbal and written counseling as appropriate.
  • Responsible for management of field personnel and coordination staff
  • Responsible for ensuring adequate field staff is available.
  • Interviews prospective employees and evaluates employee performance per agency policy.
  • Provides leadership, the application of new knowledge, and encourages staff motivation. Acts as a resource for patients/clients, families, and personnel.
  • Maintains efficient daily operation of the office.
  • Work with Branch Manager to ensure all accounting functions are maintained including A/R, A/P, P/R, billing and collections
  • Conducts both formal and informal patient/client care case conferences
  • Supervises client care. This supervision includes direct and indirect monitoring of staff interactions with clients and families.
  • Ensures orientation of new staff. Coordinates ongoing in-service education.
  • Supervises and participates as needed in the development of a comprehensive plan of care. This may include client visits to ensure safe delivery of services.
  • Must be available on premises or accessible by telecommunications during operating hours.
  • Accepts other duties as assigned.

EDUCATION, EXPERIENCE, AND TRAINING
 
  • High School Diploma is required, Associate’s Degree or higher preferred
  • Have at least one year of experience in Private Duty or home health agency, primary care clinic, or acute health facility.
  • Have sufficient background knowledge and expertise in Home Care program
