We pay $81 per/routine visits and $86 per/eval visits.

What you will do:
Assess emotional factors related to terminal illness.
Assess need for counseling related to risk assessment for pathological grief.
Assess the patient / family’s psychosocial status, potential for risk of suicide and / or abuse or neglect.
Assess environmental resources and obstacles to maintaining safety.
Assess special needs related to cultural diversity including communication, space, role of family members and special traditions.
Identify developmental level of patient/family and obstacles to learning.
Assess caregiver’s ability to function adequately in care of the patient.
Consult and collaborate with the interdisciplinary team in development and revision of the plan of care.
Involve the patient/family in the plan of care, addressing questions and issues, identifying obstacles to compliance and assisting in understanding of goals of interventions.
Connect the patient, family and caregiver with community resources, as needed and assesses patient/family ability to access.
Provide social services including, but not limited to, short-term counseling, crisis intervention, assistance with education and preparation of advance directives, funeral planning issues and transfer of responsibilities regarding fiscal, legal and health care decisions.
Skills, education, and experience:
Master of Social Work degree from a school of social work accredited or approved by the Council on Social Work Education.
Current CPR, TB and health clearance and core competency per skills checklist.
At least one year of experience in social work in a health care setting.
