The impact you will make:
- Complete initial and on-going assessments and evaluation to identify the client’s level of functioning and ability to communicate, including OASIS assessment at appropriate time points.
- Combine knowledge of speech, hearing and language with planning and implementing programs for the management of communication disorders.
- Develop the plan of care in consultation with the physician and other care team members.
- Instruct other health team personnel, including Home Health Aides and the patient's family, in Speech Therapy Program.
- Participate in in-service education, utilization review and team conferences.
- Provide discharge planning, as appropriate.
- Evaluate effectiveness and outcomes of care / response to therapy interventions.

Who you are:
- Master’s or doctoral degree in Speech-Language Pathology and meets qualifications commensurate with § 484.115(n) (Condition of participation: Personnel qualifications / Speech-Language Pathologist).
- Current, active license as a Speech Pathologist by the California Board of Medical Quality Assurance.
- Core competency per skills checklist.
- At least one year of experience in speech therapy.
