Hospice Specialist/ Sales Representative

We’re seeking a motivated and relationship-driven Sales Representative to connect patients, healthcare providers, and our agency’s Hospice services. In this role, you’ll serve as a trusted liaison in the community, helping ensure smooth transitions to care while driving growth through strong partnerships.

The annual base salary range for this position at commencement of employment is expected to be between $80,000 - $95,000; however, pay offered may vary depending on multiple individualized factors, including job-related knowledge, licensure, skills, and experience.  We also have uncapped commission and a $3,000 sign-on bonus!

Area Covered: Covina Branch (Altadena, Sierra Madre, Arcadia, Glendora, San Antonio Heights, Alta Loma, Rancho Cucamonga, Upland, Ontario, Claremont, Pomona, Covina, Baldwin Park, West Covina, El Monte, Temple City, Alhambra, San Marino, Highland Park, East L.A., Montebello, City of Industry, Walnut, Chino, Chino Hills, Diamond Bar, Rowland Heights, Hacienda Heights, La Habra, Whittier, Santa Fe Springs, Brea) 

What You’ll Do
  • Build and maintain relationships with hospitals, physicians, case managers, and community partners
  • Identify patient needs and coordinate appropriate home health, hospice, and caregiver services
  • Act as a liaison between referral sources and the agency to support seamless patient transitions
  • Educate healthcare professionals on agency services through meetings, presentations, and events
  • Participate in discharge planning and care coordination discussions
  • Execute territory plans, manage accounts, and consistently meet or exceed sales goals
  • Collaborate with clinical and marketing teams to ensure effective service delivery and outreach
  • Represent the agency at networking events and community activities
What You Bring
  • High School Diploma required; Associate degree or higher preferred
  • 2+ years of outside sales or client service experience (healthcare experience a plus)
  • Proven track record of meeting or exceeding sales goals
  • Strong communication, relationship-building, and organizational skills
  • Ability to work independently and manage a dynamic schedule
Work Environment
  • Field-based role with local travel, including hospitals and community settings
  • Requires standing, walking, and occasional lifting
  • Must have valid driver’s license, auto insurance, and a safe driving record
