LVN Patient Care Coordinator (Hospice)

We're looking for a Patient Care Coordinator to help ensure exceptional care for hospice patients and their families. In this vital role, you'll coordinate patient services, support our interdisciplinary care team, and help deliver the highest quality end-of-life care.

The hourly range for this position at commencement of employment is expected to be between $32 - $36; however, pay offered may vary depending on multiple individualized factors, including job-related knowledge, licensure, skills, and experience.

What You'll Do
  • Coordinate daily hospice patient schedules and services
  • Manage patient referrals, admissions, and intake processes
  • Obtain physician orders and verify insurance benefits/authorizations
  • Support interdisciplinary team meetings and coordinate patient care plans
  • Monitor documentation for quality, accuracy, and regulatory compliance
  • Prepare and submit required hospice quality and HIS data
  • Serve as a resource for clinical staff regarding hospice patient care
  • Collaborate with case managers to ensure continuity of care
  • Assist with staff orientation, education, and performance improvement initiatives
  • Maintain patient confidentiality and uphold hospice standards of excellence
Qualifications
  • Current California LVN or RN license preferred
  • Minimum 2 years of hospice or home health patient coordination experience
  • Knowledge of California hospice regulations and Medicare/Medicaid guidelines
  • Current CPR certification
  • Strong communication, organization, and multitasking skills
  • Proficient with computers and electronic medical records
