We are seeking an experienced and compassionate clinical leader to oversee our home health patient care services. As the Director of Patient Care Services in our Mission Hills Branch, you will lead clinical operations, ensure high-quality patient outcomes, and support a collaborative, patient-centered care environment. This is an opportunity to make a meaningful impact while guiding a dedicated healthcare team.
The annual salary range for this position at commencement of employment is expected to be between $120,000 - $140,000; however, pay offered may vary depending on multiple individualized factors, including job-related knowledge, licensure, skills, and experience.
Director of Patient Care Services (DOPCS) / Clinical Manager Reports to: Branch Manager
Key Responsibilities
Lead and oversee all home health clinical services, ensuring quality, compliance, and patient satisfaction
Coordinate patient care activities, including referrals, staffing, and care planning
Supervise clinical staff and contracted providers; provide coaching, training, and performance management
Ensure individualized care plans are developed, implemented, and evaluated in collaboration with patients and care teams
Maintain compliance with all federal, state, and accreditation standards
Monitor documentation, clinical outcomes, and quality improvement initiatives (QAPI)
Facilitate communication among physicians, clinicians, patients, and families
Conduct case conferences, in-services, and staff meetings to support ongoing development
Build and maintain strong relationships with community partners and healthcare organizations
Qualifications
Active Registered Nurse (RN) license in California
Bachelor’s degree in Nursing or related field (preferred)
3+ years of recent experience in home health, primary care, or a healthcare facility
At least 1 year in a supervisory or leadership role
Strong knowledge of home health regulations (e.g., Title 22, accreditation standards)
Proven leadership, communication, and clinical decision-making skills
Work Environment
Primarily office-based with some travel and field interaction
Requires standing, walking, and occasional off-site visits
Must maintain valid driver’s license, auto insurance, and safe driving record
