The impact you will make:
Complete initial and on-going assessments and evaluation to identify the client’s level of functioning and ability to communicate, including OASIS assessment at appropriate time points.
Combine knowledge of speech, hearing and language with planning and implementing programs for the management of communication disorders.
Develop the plan of care in consultation with the physician and other care team members.
Instruct other health team personnel, including Home Health Aides and the patient's family, in Speech Therapy Program.
Participate in in-service education, utilization review and team conferences.
Provide discharge planning, as appropriate.
Evaluate effectiveness and outcomes of care / response to therapy interventions.

Who you are:
Master’s or doctoral degree in Speech-Language Pathology and meets qualifications commensurate with § 484.115(n) (Condition of participation: Personnel qualifications / Speech-Language Pathologist).
Current, active license as a Speech Pathologist by the Arizona Board of Medical Quality Assurance.
Core competency per skills checklist.
At least one year of experience in speech therapy.
