Home Health Care Coordinator

Covina, CA
We are currently looking for a Home Health Care Coordinator for our Covina branch! LVN Preferred but we are open to non-licensed candidates if you have the right experience.

The hourly range for this position at commencement of employment is expected to be between $28-30 however, pay offered may vary depending on multiple individualized factors, including job-related knowledge, skills, and experience.

What You'll Do
Keep our home health team running smoothly — managing schedules, coordinating patient care, and making sure nothing falls through the cracks. You'll be the hub connecting patients, clinical staff, physicians, and insurance payors to deliver seamless care.

Day-to-Day Responsibilities
  • Manage scheduling for all home care services and nursing on-call rotations
  • Handle intake referrals and follow up on patient concerns
  • Communicate with physicians to receive updated orders
  • Request and manage authorizations with third-party payors
  • Participate in weekly case conferences and team meetings
  • Support field staff with nursing care functions as needed
  • Uphold patient rights, privacy, and confidentiality at all times
What We're Looking For
  • Licensed Vocational Nurse (*Preferred but not mandatory)  licensed by the California Board of Vocational Nurse and Psychiatric Technician Examiners
  • Organized, reliable, and cool under pressure
  • Strong communicator — written and verbal
  • Professional and people-oriented in every interaction
  • Committed to quality care and continuous improvement
  • Valid auto insurance and a safe driving record required
Work Environment
Primarily office-based with some walking, standing, and occasional off-site visits. Possible exposure to communicable diseases.
