We are currently looking for a Home Health Care Coordinator for our Covina branch! LVN Preferred but we are open to non-licensed candidates if you have the right experience.
The hourly range for this position at commencement of employment is expected to be between $28-30 however, pay offered may vary depending on multiple individualized factors, including job-related knowledge, skills, and experience.
Home Health Care Coordinator What You'll Do Keep our home health team running smoothly — managing schedules, coordinating patient care, and making sure nothing falls through the cracks. You'll be the hub connecting patients, clinical staff, physicians, and insurance payors to deliver seamless care.
Day-to-Day Responsibilities
Manage scheduling for all home care services and nursing on-call rotations
Handle intake referrals and follow up on patient concerns
Communicate with physicians to receive updated orders
Request and manage authorizations with third-party payors
Participate in weekly case conferences and team meetings
Support field staff with nursing care functions as needed
Uphold patient rights, privacy, and confidentiality at all times
What We're Looking For
Licensed Vocational Nurse (*Preferred but not mandatory) licensed by the California Board of Vocational Nurse and Psychiatric Technician Examiners
Organized, reliable, and cool under pressure
Strong communicator — written and verbal
Professional and people-oriented in every interaction
Committed to quality care and continuous improvement
Valid auto insurance and a safe driving record required
Work Environment Primarily office-based with some walking, standing, and occasional off-site visits. Possible exposure to communicable diseases.
