Assisted Hospice is looking for a full-time RN Case Manager for our Mission Hills branch.
Minimum of 20 visits per week. Previous hospice experience is required. 

Please note, we pay per visit. See rate of pay below. 

Regular visit rates:
Admission- $200 per visit + $11 for mileage
Routine- $70 per visit + $11 for mileage

 On-call rates:
Weekday waiting to engage rate- $50 per day 
Weekend waiting to engage rate- $100 per day
Admission- $200 per visit + $11 for mileage
Routine- $105 per visit + $11 for mileage

What you will do:
Have flexible hours and working close to home (reduce your commute!)

• Complete initial and on-going comprehensive assessments of the patient’s needs
• Provide service and /or treatments requiring substantial and specialized nursing skills
• Initiate appropriate preventative and rehabilitative nursing procedures
• Counsel, support and instruct the patient, family and caregiver in meeting nursing and related needs

Who you are:

• Graduate of an approved school of professional nursing
• Current, active license in the state of California Board of Nursing, 
• Current CPR, health clearance, TB clearance.
