JOB SUMMARY Responsible for providing and coordinating home health patient care / services consistent with the mission, vision and goals of the Agency. Speech Pathologist (SLP) is a professional member of the home health interdisciplinary team. The SLP evaluates, identifies problems, sets goals, recommends and implements mechanisms to enhance the patient’s speech, language, hearing and swallowing ability in accordance with a written plan of care prescribed by a physician.
DUTIES AND RESPONSIBILITIES
Complete initial and on-going interdisciplinary assessments and evaluation with an emphasis on the client’s level of functioning and ability to communicate, including OASIS assessment at appropriate time points.
Combine knowledge of speech, hearing and language with planning and implementing programs for the management of communication disorders.
Provide services that are ordered by the physician as indicated in the plan of care.
Develop and evaluate the plan of care in partnership with the patient, representative (if any), and caregiver(s), and in consultation with the physician and other interdisciplinary team members.
Communicate with all physicians involved in the plan of care and other health care practitioners (as appropriate), including Clinical Supervisor and interdisciplinary team members related to the current plan of care, patient’s reaction to treatment and changes in condition.
Provide education and counseling based on individualized need for the patient, family and caregiver(s).
Evaluate the home environment and make the appropriate recommendations.
Instruct other health team personnel, including Home Health Aides and the patient's family, in Speech Therapy Program.
Consult and coordinate with other health care disciplines.
Participate in Agency-sponsored in-service education, utilization review and team conferences.
Provide discharge planning, as appropriate.
Evaluate effectiveness and outcomes of care / response to therapy interventions.
Assist in developing and updating the plan of care.
Prepare and submit, as directed, all case documentation, including evaluation data, treatments, clinical and progress notes and required payroll information according to Agency policy.
Provide services within the Agency's catchment area.
Acknowledge patient rights including privacy and confidentiality.
Consistently display a professional appearance and conduct in all settings.
Interact with internal and external customers using a positive and professional approach.
Assume responsibility and display reliability for the completion of all tasks, duties, communications and actions.
Maintain professional growth and development.
Participate in the Agency’s QAPI Program for performance improvement activities.
Abide by Agency policies and procedures.
Accept other duties as assigned.
PHYSICAL DEMANDS AND WORKING ENVIRONMENT Must have visual and hearing acuity. Must have clear verbal communication. Must be able to read, write and speak English fluently. May be exposed to minor hazards/communicable diseases. Must provide proof of auto insurance upon hire and each renewal and maintain a safe driving record.
EDUCATION, EXPERIENCE, AND TRAINING
Master’s or doctoral degree in Speech-Language Pathology and meets qualifications commensurate with § 484.115(n) (Condition of participation: Personnel qualifications / Speech-Language Pathologist).
Current, active license as a Speech Pathologist by the California Board of Medical Quality Assurance.
