We are looking for an LVN Patient Care Coordinator for our Hospice department in Santa Barbara. This is a temporary role which might possibly turn into something permanent; however, the initial opportunity expects to be a 6-month plus time frame.
The hourly range for this position at commencement of employment is expected to be between $34-$38 per/hour; however, pay offered may vary depending on multiple individualized factors, including job-related knowledge, licensure, skills, and experience.

JOB SUMMARY: This position serves as part of the hospice interdisciplinary team under the direction and supervision of the Clinical Supervisor of Patient Care Services. The Coordinator is responsible for all matters pertaining to the daily coordination, evaluation of patient/family services provided by the hospice interdisciplinary team consistent with the mission, vision and goals of the Agency.
DUTIES AND RESPONSIBILITIES
Determines and maintains schedule of all hospice services.
Receives updated orders from physicians.
Performs all duties and responsibilities related to the intake and referral process.
May assist with the orientation of new personnel.
Promotes staff development.
Participates in care plan development, coordination of patient services, continuity of care and discharges.
Coordinates and prepares information for interdisciplinary team meetings.
Requests authorization of utilization from third party payors and verifies patient benefits as necessary.
Provides a detailed report to Case Managers of third party payors..
Performs nursing care and other staff functions as necessary.
Performs concurrent chart reviews.
Reviews hospice nursing notes for quality and accuracy.
Prepares & inputs HIS and quality outcome data.
Acts as a resource for staff concerning patient care issues relating to hospice.
Acknowledges patient rights including privacy and confidentiality.
Consistently displays a professional appearance and conduct in all settings. Interpersonal and interagency relationships are to be positive and professional at all times.
Assumes responsibility and displays reliability for the completion of all tasks, duties, communications and actions.
Maintains professional growth and development through networking, membership in professional organizations and continuing education.
Participates in performance improvement activities.
Abides by Agency policy and procedures.
Accepts other duties as assigned.
PHYSICAL DEMANDS AND WORKING ENVIRONMENT Work is mostly sedentary combined with moderate amounts of walking and standing. Must have visual and hearing acuity. Must be able to clearly communicate verbally. Must be able to read, write and speak English fluently. Must be able to lift weight equivalent to a box of computer paper. Attends and participates in activities off-site as necessary. May be exposed to minor hazards or communicable diseases. Must provide proof of auto insurance upon hire and each renewal and maintain a safe driving record.
EDUCATION, EXPERIENCE AND TRAINING
Current, active license to practice as a Licensed Vocational Nurse or RN in the State of California (preferred)
At least two years patient coordination experience in hospice or home health.
Working knowledge of California Hospice, HCFA and JCAHO standards.
