The annual salary range for this position at commencement of employment is expected to be between $120,000 - $130,000; however, pay offered may vary depending on multiple individualized factors, including job-related knowledge, licensure, skills, and experience.

What you will do:
Ensure completion of initial and ongoing interdisciplinary assessment of the patient.
Manage the daily assignment and workload of the Agency. Direct hospice staff on a daily basis to ensure appropriate assignment and use of multi-disciplinary service to all patients.
Perform nursing care and other field staff functions, as necessary.
Supervise all patient care provided. This supervision includes direct and indirect monitoring of professional staff interactions with patients, families, and physicians.
Ensure that services ordered by physicians are provided in a coordinated manner.
Communicate with all physicians involved in the plan of care and other health care practitioners (as appropriate) relating to the current plan of care.
Cover for the Director of Patient Care Services, making administrative decisions, and problem solving in Director's absence.
Take administrative "on-call" in rotation or when asked to do so.
Who you are:
Licensed to practice as a registered nurse (RN) by the Board of Registered Nursing in the state of California.
Minimum of bachelor’s degree in nursing or other health-related field with three years of experience within the last five years in a home health or hospice agency, primary care clinic or health facility.
Prior supervisory or administrative experience in home health, hospice, or hospital environment
