JOB SUMMARY
Provides home health services consistent with the mission, vision and goals of the Agency.  Occupational Therapy Assistant is a member of the home health team who assists in the provision of occupational therapy under the supervision of an Occupational Therapist to provide treatment of disabilities, diseases, or other conditions of health that is focused on developing, improving, or restoring functional daily living skills, compensating for and preventing dysfunction or minimizing disability. The use of hand therapy and physical agent modalities such as splints, or orthotics and other techniques are provided in accordance with a written plan of treatment prescribed by a physician.  The Occupational Therapy Assistant contributes to the coordination of patient care.

DUTIES AND RESPONSIBILITIES
  • Perform services planned, delegated and supervised by the Occupational Therapist.
  • Contribute to the ongoing interdisciplinary assessment of the patient.
  • Contribute to / assists with evaluation of the plan of care in collaboration with the Occupational Therapist, and in partnership with the patient, representative (if any), and caregivers.
  • Communicate with the supervising Occupational Therapist, all physicians involved in the plan of care and other health care professionals (as appropriate) related to the current plan of care, including patient reaction to treatment and changes in patient condition.
  • Assess ability or disability, relieve pain, prevent disability and/or further deformity, develop or improve body skills, and restore function and maintain maximum performance within the patient's capabilities.
  • Assess and perform procedures that may contribute to the evaluation process by gathering data, administering standardized tests and reporting observations.
  • The Occupational Therapist shall determine the duties the assistant can perform and shall consider the following: clinical complexity of the patient, skill level of the COTA and whether continual reassessment of the patient is needed during treatment.
  • The COTA may contribute to the preparation, implementation, and documentation of the treatment and discharge summary.
  • Instruct patient in use of OT equipment such as reachers, grab bars, bathing, grooming or other ADL aids, braces, splints and other prosthetic and orthotic devices.
  • Participate in teaching / counseling of the patient, family, and caregiver(s).
  • Participate in Agency-sponsored in-service education, utilization review and team conferences.
  • Prepare and submit all clinical case documentation, including clinical and progress notes, and required payroll information according to Agency policy.
  • Document each treatment in the patient’s record and notify the supervising occupational therapist of any change in the patient’s condition not consistent with planned progress or treatment goals.
  • Ensure that the supervising Occupational Therapist conducts at least weekly review and inspection of all aspects of OT services provided by the COTA.
  • COTAs may supervise OTA students and home health aides providing related tasks.
  • Provide services within the Agency's catchment area.
  • Acknowledge patient rights including privacy and confidentiality.
  • Consistently display a professional appearance. 
  • Interact with internal and external customers using positive and professional approach.
  • Assume responsibility and display reliability for the completion of all tasks, duties, communications and actions.
  • Maintain professional growth and development.
  • Participate in the Agency’s QAPI Program for  performance improvement activities.
  • Abide by Agency policies and procedures.
  • Accept other duties as assigned.

EDUCATION, EXPERIENCE, AND TRAINING
  • Education and qualifications commensurate with § 484.115(g) (Condition of participation:  Personnel qualifications / Occupational Therapy Assistant).
  • Current, active license as a Certified Occupational Therapist Assistant by the California Board of Occupational Therapy.  
  • Have graduated from an Occupational Therapy Assistant program approved by the Board and has successfully passed the examination required by the Board governing occupational therapy assistant.

PHYSICAL DEMANDS AND WORKING ENVIRONMENT
Must have visual and hearing acuity.  Communicates verbally in a clear fashion.  Must be able to read, write and speak English fluently. May be exposed to minor hazards/communicable diseases. Must provide proof of auto insurance upon hire and each renewal and maintain a safe driving record.
