The base pay range for this position at commencement of employment is expected to be between $27-30 per/hour; however, base pay offered may vary depending on multiple individualized factors, including job-related knowledge, licensure, skills, and experience.

JOB SUMMARY This position serves as part of the Home Health team under the direction and supervision of the Branch Manager. The coordinator is responsible for all matters pertaining to the daily coordination of patient/family services provided by the Home Health team consistent with the mission, vision and goals of the Agency.
DUTIES AND RESPONSIBILITIES
Maintain schedule of all Home Health services.
Assists with preparation of nursing call schedule.
Staff patient referrals with MD ordered clinicians
Takes and prepares intake referrals.
Participates in performance goals of the Agency.
Participates in Interdisciplinary Group meetings when available.
Verify Medicare and Insurance benefits as requested
When requested, give a report to case managers of third-party payors.
Acknowledges patient rights including privacy and confidentiality.
Consistently displays a professional appearance and conduct in all settings. Customer service, including interpersonal and interagency relationships, are to be positive and professional at all times.
Assumes responsibility and displays reliability for the completion of all tasks, duties, communications and actions.
Supports the mission, vision and goals of the Agency.
Maintains professional growth and development.
Participate in performance improvement activities.
Abides by Agency policies and procedures.
Accepts other duties as assigned.
Accepts overtime if required
PHYSICAL DEMANDS AND WORKING ENVIRONMENT Work is mostly sedentary combined with moderate amounts of walking, standing, lifting, and climbing stairs. Must have visual and hearing acuity. Must be able to communicate verbally and in writing in English. Attends and participates in activities off site as necessary. May be exposed to minor hazards or communicable diseases. Must provide proof of auto insurance upon hire and each renewal and maintain a safe driving record.
EDUCATION, EXPERIENCE, AND TRAINING
At least two years of patient coordination experience or equivalent in-home health, hospital or medical group.
Working knowledge of Medicare Conditions of Participation
