JOB SUMMARY
This position requires clinical expertise in home care, effective communication skills, knowledge of adult teaching- learning principles, and patience. The SDC-Field Educator will perform joint visits with agency staff to provide guidance and instruction, validate competency, and model professionalism and customer service. He or she will serve as the extended support service necessary for executing an effective staff education program supporting Performance Improvement activities consistent with the Mission and Goals of the Agency.
What you will do:
- Provide technical support and instruction to all levels of staff relating to patient care issues, teaching/ learning and Performance Improvement projects in home health and/or hospice.
- Perform joint visits with new employees during the orientation period to reinforce clinical, documentation, communication skills and expectations.
- Perform OASIS validation assessments (home health) with clinicians in the office and the field to ensure OASIS Competency.
- Assist with Point of Care Documentation (computer) training and follow-through implementation in the field.
- Perform joint visits with current employees needing evaluation or guidance, as needed and as requested.
- Provide constructive feedback to staff following joint visits.
- Coordinate and document all joint visit experiences, observations, and suggestions.
- Communicate observations and outcomes of joint visits with staff supervisor/manager.
- Assist Director of Education or SDC designee in planning and implementation of new employee field orientation.
- Assist in presenting Agency classroom orientation to support or fill-in for SDC, when requested
- May participate, as requested, in development of written staff educational materials for all levels of staff.
- Contribute observations and data regarding joint visits, orientation, training and competency issues, as appropriate and as requested, to the Director of Education.

Who you are:
- Licensed to practice as a Registered Nurse by the Board of Registered Nursing in the State of California.
- Bachelor's degree in nursing preferred, with at least 2 years of experience in home health or hospice. Staff Development or teaching experience desired.
- Must provide proof of automobile insurance upon hire and at each renewal and maintain a safe driving record (driving to multiple location is required)
- Current CPR, health clearance and clinical skills self-assessment.
- Successful passage (80%) of Agency competency exam and core competency per skills checklist
- Working knowledge of State and Federal regulations including accreditation standards.
