We’re looking for a full-time RN Clinical Care Coordinator to support our home health division in Los Angeles.
This is an office position, working Monday through Friday, 8am to 5pm. No hybrid or remote options.
$42- $45 per hour
QI/ QA experience is strongly preferred

What you will do:
  • Ensures completion of initial and ongoing interdisciplinary assessment of the patient.
  • Manages the daily assignment and workload of the Agency.  Directs home health staff on a daily basis to ensure appropriate assignment and use of multi-disciplinary service to all patients.
  • Ensures that services ordered by physicians are provided in a coordinated manner.
  • Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) relating to the current plan of care.
  • Conducts both formal and informal conferences to ensure that patient care is provided in the prescribed manner. 
  • Orients new staff to the field. Coordinates, and participates in Agency-sponsored in-service training / education programs.
  • Participates in the Agency’s QAPI Program in collaboration with the Director of Patient Care Services for performance improvement initiatives.
  • Instruct home health care staff regarding a specialized skill, assist with problem cases
  • Prepares clinical notes to document patient care and interventions rendered.

Who you are:
  • Graduate of an approved professional nursing program.
  • Registered Nurse licensed by the California Board of Registered Nursing.
  • At least two years of patient coordination experience or equivalent in-home health, hospital or medical group.
  • Working knowledge of Medicare Conditions of Participation
  • Current CPR card.
  • Core competency per skills checklist.
  • Current health clearance.
